目的:探讨外周血相关炎性指标对糖尿病性视网膜病变(diabetic retinopathy,DR)患者继发新生血管性青光眼(neovascular glaucoma,NVG)的影响。 方法:回顾性的病例对照研究。根据患者有无NVG分为两组:单纯增殖性糖尿病性视网膜病变(Proliferative diabetic retinopathy,PDR)患者(单纯PDR组,n=148)和继发于PDR的NVG患者(PDR继发NVG组,n=142),记录两组患者的外周血炎性指标水平,包括白细胞相关炎症指标、中性粒细胞与淋巴细胞比值、血小板与淋巴细胞比值、单核细胞与淋巴细胞比值以及全身免疫炎症指数,分析两组患者外周血炎性指标的差异及与PDR继发NVG的关系。 结果:两组患者基本特征无统计学差异,具有可比性。单纯PDR组与PDR继发NVG组患者嗜酸性粒细胞百分比、单核细胞与淋巴细胞比值之间存在统计学差异(P<0.05),PDR继发NVG组明显高于单纯PDR组患者。通过多因素logistic回归分析显示嗜酸性粒细胞百分比、单核细胞与淋巴细胞比值是PDR患者继发NVG的影响因素。 结论:嗜酸性粒细胞百分比、单核细胞与淋巴细胞比值可能与PDR患者进展为NVG有关,可能对PDR患者继发NVG有预测作用。

Objective: To investigate the influence of relevant inflammatory markers in peripheral blood on the progression of neovascular glaucoma (NVG) secondary to diabetic retinopathy (DR) among patients. Methods: A retrospective case-control study was conducted, categorizing patients into two groups based on the presence or absence of NVG: those with proliferative diabetic retinopathy (PDR) alone (PDR group, n=148) and those with NVG secondary to PDR (NVG secondary to PDR group, n=142). Peripheral blood inflammatory markers were evaluated, including white blood cell-related indices, neutrophil-to-lymphocyte ratio (NLR), platelet-to-lymphocyte ratio (PLR), monocyte-to-lymphocyte ratio (MLR), and systemic immune-inflammation index (SII). Analyze the distinctions in peripheral blood inflammatory markers between the two groups of patients and the relationships these bear to NVG secondary to PDR. Results: No statistically significant differences were observed in baseline characteristics between the two groups, confirming their comparability. However, significant differences were found in eosinophil percentage and monocyte-to-lymphocyte ratio (MLR) between the PDR group and the NVG secondary to PDR group (P < 0.05), with both values being significantly higher in the NVG secondary to PDR group. Multivariate logistic regression analysis revealed that the eosinophil percentage and the monocyte - to - lymphocyte ratio were factors influencing the development of neovascular glaucoma (NVG) secondary to proliferative diabetic retinopathy (PDR) in patients. Conclusion: Eosinophil percentage and MLR may be associated with the progression of PDR to NVG and could serve as potential predictive markers for NVG development in PDR patients.
