Abstract:With the continuous transformation of the digital era and the ongoing collaboration of interdisciplinary fields, specialized applications of artificial intelligence(AI)have already made significant strides in the field of medicine. Given the precision required in ophthalmology and its high demands for intelligent solutions, particularly in the realm of nursing care, this study aims to explore the clinical practices and current status of AI in ophthalmic nursing based on a comprehensive review of domestic literature. Employing a scoping review framework, the research questions were formulated, and relevant studies were retrieved from Wanfang, the China National Knowledge Infrastructure(CNKI), VIP, PubMed, and Web of Science. The search spanned from January 2013 to December 2023. In accordance with inclusion and exclusion criteria, literature screening was conducted, and data extraction was performed on selected articles. A total of 20 articles were included in the review. The development of AI in ophthalmic nursing in China is relatively recent and not yet comprehensive. Continuous innovation in new technologies and projects is required by nursing professionals to effectively integrate with the advancements in intelligent medical care.

Abstract:AIM: To identify the primary active components and underlying mechanisms of Yijing Decoction(YJD)in treating early diabetic retinopathy(DR)based on liquid chromatography-mass spectrometry and network pharmacology.
METHODS: Active components of YJD were characterized through LC-MS. Components with optimal ADME(absorption, distribution, metabolism, excretion)properties were selected as key bioactive candidates. Network pharmacology approaches were employed to predict YJD-DR therapeutic targets. Protein-protein interaction(PPI)networks, gene ontology(GO)enrichment analysis, and Kyoto Encyclopedia of Genes and Genomes(KEGG)pathway analysis were subsequently conducted to predict core targets and networks. Critical targets and pathways were experimentally validated through Western blot.
RESULTS: Ten core therapeutic targets were identified, including TNF, Alb, EGFR, STAT3, PTGS2, ESR1, PPAR, MMP9, TLR4, and MAPK. YJD was related to cancer-related signaling, fluid shear stress and atherosclerosis, and neurodegenerative diseases, encompassing key biological processes such as inflammatory response regulation, programmed cell death activation, and enhanced cell migration. Furthermore, Western blot analysis confirmed that YJD significantly inhibited high glucose-induced phosphorylation of STAT3(P-STAT3/STAT3)and ERK(P-ERK/ERK)in rat retinal microvascular endothelial cells.
CONCLUSION: This study revealed YJD's pharmacodynamical basis and its multi-component, multi-target, and multi-paths pharmacology. YJD exerts therapeutic effects on DR by coordinately regulating critical signaling pathways and alleviating intraocular inflammation, thus preserving retinal vascular endothelial cells, maintaining blood-retinal barrier integrity, and facilitating retinal neurovascular repair.

Abstract:AIM: To investigate the specific molecular mechanism of Yinqiao Powder in affecting macrophage polarization in herpes simplex keratitis(HSK)through the cyclic GMP-AMP synthetase(cGAS)-stimulator of interferon genes(STING)-interferon regulatory factor 3(IRF3)molecular pathway.
METHODS:Human corneal epithelial cells(HCE-T)were divided into control, HSK, and HSK + Yinqiao Powder groups. M0 macrophages were grouped as Ctrl, HSV-1, HSV-1+oe-cGAS, HSV-1+Yinqiao Powder, and HSV-1+oe-cGAS+Yinqiao Powder. Conditional medium(CM)from each group of M0 macrophages was collected to intervene in HCE-T cells and divided into Ctrl-CM, HSV-1-CM, HSV-1+oe-cGAS-CM, and HSV-1+Yinqiao Powder-CM groups. Cell viability was detected by MTT assay, and apoptosis was detected by TUNEL assay. ELISA was used to detect the concentrations of Arg-1 and iNOS in cell supernatants, and Western blotting was used to detect the relative protein expressions of cGAS, STING, and IRF3. Balb/c mice were divided into control, model, and drug groups. The model and drug groups were inoculated with HSV-1 on the cornea of Balb/c mice using the corneal scratch method to construct an HSK mouse model, and the drug group was treated with Yinqiao Powder. The incidence and mortality of the three groups were compared on days 1, 3, 5, 7, and 14 after modeling.
RESULTS:Compared with the control group, the HCE-T cell viability in the HSK group was decreased but apoptosis was increased, which was reversed by Yinqiao Powder intervention. Compared with the Ctrl group, the Arg-1 concentration in the cell supernatant of the HSV-1 group was decreased, the iNOS concentration was increased, and the protein expressions of cGAS, STING, and IRF3 were decreased. Compared with the HSV-1 group, the Arg-1 concentration was increased, the iNOS concentration was decreased, and the protein expressions of cGAS, STING, and IRF3 were enhanced in the HSV-1+oe-cGAS group and the HSV-1+Yinqiao Powder group, and the same results were obtained in the HSV-1+oe-cGAS+Yinqiao Powder group. Compared with the Ctrl-CM group, the HCE-T cell viability was decreased and apoptosis was increased in the HSV-1-CM group, which was reversed by overexpressing cGAS in macrophages or intervening with Yinqiao Powder. In vivo experiments found that Yinqiao Powder intervention could improve the pathological progression of keratitis.
CONCLUSION:Yinqiao Powder inhibits M1 polarization of macrophages through the cGAS-STING-IRF3 molecular pathway, thereby delaying the progression of HSK.

Abstract:AIM:To investigate the synergistic efficacy of travoprost and betahistine mesylate in early open angle glaucoma.
METHODS:This study is a prospective, randomized, controlled, open label single center clinical trial that enrolled 82 patients(82 eyes)with early primary open-angle glaucoma from January 2020 to January 2023(eligible eyes were included, and the right eye was selected if both eyes met the inclusion criteria). The patients were randomly divided into a monotherapy group(41 eyes)treated with only travoprost eye drops and a combination therapy group(41 eyes)treated with travoprost eye drops and oral betahistine mesylate according to a 1:1 ratio using a random number table method. Followed-up for 12 mo, the intraocular pressure(IOP), retinal nerve fiber layer(RNFL)thickness, ocular hemodynamics \〖peak systolic velocity(PSV), end diastolic velocity(EDV), resistance index(RI), pulsatility index(PI)\〗, liver and kidney function, quality of life, and clinical symptom scores before and after treatment were compared between the two groups of patients.
RESULTS:Totally 4 patients were unable to complete all follow-up visits due to various factors, including 2 cases in the monotherapy group and 2 cases in the combination therapy group, with a lost rate of follow-up of 5%. The IOP in the combination therapy group was significantly lower than that in the monotherapy group at all time points(all P<0.05). Additionally, the rate of reduction in RNFL thickness was significantly slower in the combination therapy group compared to the monotherapy group(all P<0.05). Hemodynamic parameters revealed that PSV and EDV were significantly higher in the combination therapy group at 12 mo, while RI and PI were significantly lower than those in the monotherapy group(all P<0.05). The quality of life scores and visual analog scale(VAS)scores were also significantly better in the combination therapy group compared to the monotherapy group(all P<0.05). There were no statistically significant differences in liver functions, including alanine aminotransferase(ALT), aspartate aminotransferase(AST), total bilirubin(TBIL), and kidney functions, including serum creatinine(Scr), blood urea nitrogen(BUN), and uric acid(UA)between the two groups at 6 and 12 mo after treatment, with no serious damage to liver and kidney or other systemic adverse reactions observed in either groups. Furthermore, the incidence of headache in the combination group was lower than that of the monotherapy group(P=0.042), and there were no statistical significance in the incidence of other adverse reactions(all P>0.05).
CONCLUSION:The combination therapy of travoprost and betahistine mesylate exhibits significant synergistic effects in patients with early primary open angle glaucoma, offering better IOP control, neuroprotection of the optic nerve, and oxidative stress inhibition. This combination may provide a new clinical reference for comprehensive glaucoma treatment.

Abstract:AIM: To predict diabetic retinopathy(DR)progression through macular layer thickness in diabetic patients detected by optical coherence tomography(OCT).
METHODS: Retrospective study. The clinical data of 100 cases(200 eyes)of diabetic patients admitted to our hospital from January 2023 to September 2024 were collected. According to the international clinical DR classification, they were divided into the non-diabetic retinopathy(NDR)group with 32 cases(64 eyes), the non-proliferative diabetic retinopathy(NPDR)group with 38 cases(76 eyes), and the proliferative diabetic retinopathy(PDR)group with 30 cases(60 eyes). At the same time, 49 cases(98 eyes)of healthy controls whose age and gender were matched with those of the diabetic patients were collected as the normal group. All patients underwent OCT examination. The thickness changes of the retinal nerve fiber layer(RNFL), ganglion cell layer(GCL), inner plexiform layer(IPL), outer nuclear layer(ONL), photoreceptor cell layer and total retinal thickness(RT)in the subregions of the macular area were compared among the groups. The Eta coefficient was used to analyze the correlation between them and the severity of DR.
RESULTS: The thickness of RNFL, GCL, IPL, ONL and photoreceptor cell layer in each sub-region and the average of macular area in the PDR group was significantly lower than that in the NDR and normal groups, while the average RT thickness was significantly higher than that in the NPDR, NDR and normal groups(all P<0.05). The thickness of RNFL(central area, upper inner and outer rings and lower inner and outer rings and average), GCL(upper inner and outer rings and lower inner and outer rings and average), IPL(upper inner ring), ONL(central, upper inner ring and lower inner ring)and photoreceptor cell layer(upper inner and outer rings and lower inner and outer rings and average)in macular area of the PDR group was significantly thicker than that in the NPDR group(all P<0.05). The thickness of RNFL, GCL, IPL, ONL and photoreceptor cell layer in each sub-region and the average of macular area in the NPDR group was significantly lower than that in the NDR and normal groups, while the average RT thickness was significantly thicker than that in the NDR and normal groups(all P<0.05). There was no statistically significant difference in the above indicators between the NDR group and the normal group(all P>0.05). The severity of DR was significantly correlated with the average thickness of RNFL, GCL, IPL, ONL, photoreceptor cell layer and RT in macular area(all P<0.001).
CONCLUSION: OCT measurement of the thickness of RNFL, GCL, IPL, ONL, photoreceptor cell layer and RT in the macular area in the diabetic patients can evaluate the progression of DR.

Abstract:The ocular fundus vasculature, serving as a critical window for monitoring disease progression, represents one of the primary targets of hypoxic injury. A growing body of evidence suggests associations between specific ocular vascular pathologies and sleep-disordered breathing. Obstructive sleep apnea(OSA)has been implicated in fundus lesions through its detrimental effects on the central retinal artery, retinal veins, retinal microvasculature, and choroidal vessels. Mechanistically, these effects are linked to OSA-induced intermittent hypoxia, which drives hemodynamic disturbances, oxidative stress, inflammatory responses, altered blood composition, endothelial dysfunction, and neuroendocrine/metabolic dysregulation. This review synthesizes current evidence on OSA-related retinal vascular injury and elucidates its mechanistic pathways. The goal is to identify sensitive and specific retinal vascular biomarkers to facilitate the early detection of OSA and its associated complications.

Abstract:With the younger age of electronic device use and changes in lifestyle, the incidence of dry eye in children has significantly increased, becoming a research hotspot in clinical and scientific fields. Due to the dissociation of symptoms and signs, hidden manifestations, and complex etiology, children with dry eye form a special diagnostic and treatment group. Traditional dry eye detection methods have defects such as low cooperation from children, poor accuracy, and invasiveness, and the lack of a unified diagnostic guideline makes it difficult to accurately assess relevant indicators, urgently requiring scientific diagnostic technologies and standards. In recent years, new detection technologies have brought breakthroughs. The Keratograph 5M can objectively evaluate tear film stability and meibomian gland function; questionnaires such as DEQ-5 enhance the feasibility of subjective symptom feedback in young children; SM Tube test strips, with their rapid and non-invasive advantages, have become efficient tear screening tools; and the application of artificial intelligence(AI)has further revolutionized the diagnostic model, significantly improving diagnostic efficiency and children's compliance. However, existing technologies still face challenges such as difficulty in grassroots popularization, lack of child-specific reference values, and insufficient interdisciplinary data integration. Future efforts should focus on establishing age-stratified diagnostic criteria through multi-center collaboration, integrating AI with multimodal detection technologies, and constructing a diversified evaluation system to support early intervention and precision treatment for childhood dry eye. This paper systematically reviews the progress in detection technologies for childhood dry eye, focuses on the uniqueness of children as a special diagnostic and treatment group, discusses clinical difficulties and challenges, and integrates multimodal and intelligent methods to provide innovative solutions and practical pathways for precise diagnosis, reduction of misdiagnosis rates, and improvement of the diagnostic and treatment system for childhood dry eye.

Abstract:Apparent accommodation originates from the residual accommodation function after the absence of classic lens accommodation, serving as a critical theoretical framework in the study of visual function after refractive cataract surgery. Different from Von Helmholtz's theory, which focuses on lens deformation, it evaluates the overall accommodation capability of the eye excluding the lens, with its mechanisms remaining debated. Apparent accommodation refers to the character that pseudophakic eyes retain a certain degree of refractive accommodation after natural lens removal or intraocular lens(IOL)implantation, despite the loss of physiological accommodation. This paper systematically reviews the historical development of apparent accommodation and the advances in assessment techniques, which have gradually contributed to the re-evaluation and expansion of classical theories of physiological accommodation. Recent studies attempt to decipher the synergistic effects between pupil, corneal, and central cognitive processing by integrating optical parameter measurements with neural adaptation. This reflects a shift in the field from a singular anatomical explanation to a more interdisciplinary and multi-dimensional model.

Abstract:Central retinal vein occlusion(CRVO)is a retinal vascular disorder that significantly impairs vision, with its underlying mechanisms involving complex interactions across multiple biological systems. This article provides a systematic review of the pathological mechanisms associated with CRVO, emphasizing critical factors such as endothelial dysfunction, arteriosclerosis, thrombophilia, inflammation, and oxidative stress. The pathological mechanisms of CRVO are characterized by arteriosclerosis, which obstructs venous return through a dual mechanism involving mechanical compression and endothelin-1-mediated contraction; endothelial dysfunction, which exacerbates disturbances in blood flow; genetic and acquired coagulation abnormalities that disrupt hemostatic balance and promote thrombosis; and the synergistic effects of inflammation and oxidative stress that activate cytokines, thereby aggravating ischemia and vascular leakage. Innovatively, this review explores emerging mechanisms such as miRNA-mediated vascular regulation via exosomes, gut microbiota-retina crosstalk through the “gut-eye axis,” and systemic metabolic interactions that link local retinal lesions to broader dysregulation of CRVO. These insights underscore the importance of integrated eye-system interventions and provide a theoretical foundation for advancing early biomarker discovery, multitarget therapeutics, and personalized treatment paradigms. By bridging localized pathology and systemic mechanisms, this work promotes a transformative shift toward an integrative medicine model in the diagnosis and management of CRVO.

Abstract:Intravitreal injection of anti-vascular endothelial growth factor(VEGF)agents has become the primary treatment for macular edema associated with retinal vascular disease such as diabetic retinopathy and retinal vein occlusion, but there are limitations such as variable treatment efficacy and insufficient durability of therapeutic effects. As the first bispecific antibody applied in ophthalmic treatment, Faricimab achieves favorable outcomes by simultaneously targeting both VEGF-A and angiopoietin-2(Ang-2)pathways. Based on evidence from recent clinical trials and real-world studies, this article reviews the research progress on Faricimab for the treatment of diabetic macular edema(DME), retinal vein occlusion-associated macular edema(RVO-ME)and refractory macular edema compared to the therapeutic effects of other agents. Additionally, based on Faricimab's safety characteristics and future potential, its therapeutic prospects for macular edema associated with retinal vascular diseases are discussed. This review aims to provide evidence-based references for optimizing clinical treatment strategies, thereby contributing to mitigating the risk of vision loss due to macular edema.

Abstract:Fractals refer to structures whose component parts exhibit similarity to the whole in certain aspects. The retinal microvascular system, as the only terminal microvasculature that can be directly observed in vivo, possesses a tree-like branching morphology that conforms to the characteristics of fractals. Fractal dimension(FD)is a numerical value that describes the density and complexity of the overall retinal vascular network, complementing the limitations of vessel density alone in characterizing vascular structural features. In recent years, the widespread application of optical coherence tomography angiography(OCTA)has enabled the visualization of blood flow across various retinal capillary layers, thereby extending the concept of fractal analysis to the retinal microvasculature. FD has been demonstrated to serve as a novel potential biomarker for ophthalmic conditions, including diabetic retinopathy, glaucoma, age-related macular degeneration, high myopia, and retinal vein occlusion, providing valuable metrics for the early diagnosis of these diseases. This review provides a comprehensive summary of the definition, calculation methods, influencing factors, and recent research developments regarding FD in various ophthalmic disorders.

Abstract:Radiation retinopathy(RR)is a chronic and progressive retinal vascular disease that occurs during radiotherapy for tumors. It is commonly seen after radiotherapy for head and neck tumors, especially those close to the eye. The incidence and severity of RR are influenced by risk factors such as radiation location, radiation type, and radiation dose. The basic pathological change of RR is the damage to vascular endothelial cells which could lead to maculopathy and subsequent vision loss. The interventions for RR include prophylaxis and treatment. Prophylaxis has been proved effective for reducing the occurrence of RR. Intravitreal injection of anti-vascular endothelial growth factor(VEGF)agents was the first-line therapy for treating RR, and corticosteroids and laser photocoagulation could serve as supplements. This article reviewed the risk factors, pathogenesis, clinical manifestations, and diagnostic and treatment methods of RR, aiming to enhance the understanding of RR and summarize the recent advances in RR.

Abstract:With the application and popularization of the polymerase chain reaction, many ocular diseases with unknown etiology in the past have been confirmed to be related to viral infections; from the classic herpes simplex keratitis to the cytomegalovirus intraocular infection which is gradually recognized by people at present, various viral ocular diseases have received increasing clinical attention. Ocular viral infections are complicated with various types of viruses causing different clinical manifestations. Since it is difficult to obtain corresponding tissue specimens from patients with viral ocular diseases, establishing an effective animal model is a crucial foundation for studying the characteristics of the disease course, the pathogenesis, and the pre-clinical evaluation of drugs. Based on the literature on animal models related to viral ocular diseases in recent years, this paper comprehensively summarizes the animal models of various diseases from the anterior segment to the posterior segment of the eye caused by different viruses and introduces in detail the types of viruses, viral loads, and the methods of virus challenge on various models; besides, it conducts a comparative evaluation from multiple dimensions such as the model stability, advantages and disadvantages of the models, and their application situations, so as to provide a basis for subsequent basic research on related diseases and drug transformation.

Abstract:Myopia has become a common eye disease in China and around the world. The myopia rate among children and adolescents in China has been continuously increasing, and it shows the characteristics of younger age and higher severity, seriously threatening eye health. According to recent studies, myopia is closely related to the axial length. During the progression of myopia, the excessively long axial length would lead to changes in ocular structure like the sclera, retina, and choroid, etc., thereby increasing the onset risk of diseases such as posterior staphyloma of the sclera and retinal detachment, and ultimately resulting in severe vision impairment. By reviewing the related literature at home and abroad in recent years, the paper systematically sorted out the relationship between the changes in axial length and fundus damage, deeply discussed the interacting mechanisms, aiming at providing reference for the prevention and treatment of myopia in children and adolescents, with the expectation of reducing the occurrence of complications of myopic fundus lesions, which has important practical significance for protecting the eye health of children and adolescents.

Abstract:Myopia prevalence is still rising worldwide, addressing the worldwide myopia “epidemic” is of utmost importance, since the World Health Organization estimates that half of the world's population will suffer from myopia by the middle of the century. In addition to causing vision loss, myopia is closely associated with irreversible blinding disorders such as glaucoma and retinopathy. The pathophysiology of myopia is strongly linked to the biomechanical effects of intraocular pressure(IOP)on the scleral structure. Scleral tissue undergoes considerable structural remodelling during myopia development, increasing its sensitivity to the mechanical stresses of IOP. Important mechanisms include increased collagen degradation brought on by matrix metalloproteinase(MMP-2)activity, extracellular matrix dynamics destabilization from an imbalance in the regulation of tissue inhibitor of metalloproteinase(TIMPs), aberrant choroidal blood supply brought on by activation of the hypoxia-inducible factor-1α(HIF-1α)pathway, and cascading regulation of the PI3K/AKT signaling pathway in scleral remodeling. This study provides a significant scientific foundation and fresh concepts for the future research into myopia prevention and control strategies by methodically examining the connection between changes in IOP and the development of myopia as well as its molecular underpinnings.

Abstract:In recent years, the prevalence of myopia has shown a significant upward trend characterized by earlier onset and accelerated progression rates. This epidemic not only imposes an increasing socioeconomic burden but also leads to severe vision impairment through high myopia-related complications that profoundly affect daily life. Current research on the pathogenesis of myopia primarily focuses on four mechanistic theories, including scleral remodeling, choroidal hemodynamic abnormalities, dopamine synthesis and metabolism, and inflammatory responses. The advent of high-throughput sequencing technologies has revolutionized our investigative approaches, enabling deeper exploration into myopia development through multi-omics strategies encompassing genomics, proteomics, and metabolomics. These cutting-edge methodologies have provided novel insights for myopia prevention and control, while simultaneously identifying potential therapeutic targets for precision intervention. This review focuses on summarizing the aforementioned research findings.

Abstract:Presbyopia is a physiological phenomenon related to age, in which there is a progressive difficulty in near vision due to decreased accommodation ability. It is one of the main reasons for the decline in visual related quality of life of people over 40 years old. The therapeutic objective is to improve symptoms, enhance functional near vision, and achieve the goal of restoring self-accommodation. A multitude of correction strategies are available, conventional spectacles is the simplest and safest method, which including monofocal, bifocal, trifocal, or gradient multifocal glasses. With the increasing demand for visual quality, corneal contact lenses and presbyopia correction surgeries(corneal refractive surgery, intraocular surgery, and scleral surgery)are becoming more and more accepted. The goal of these surgeries is to correct existing refractive errors while properly addressing the issue of decreased accommodation ability. At the same time, research on pharmacological interventions for presbyopia has been increasing year by year, including miotics and lens softeners, to improve the symptoms of presbyopia through different mechanisms. This article provides a review of relevant literature on presbyopia correction in recent years.

Abstract:AIM: To compare the efficacy of different intraocular lens(IOL)fixation in aphakic patients lacking capsule support.
METHODS:Retrospective study. Totally 120 cases(120 eyes)of aphakia patients who lacked capsule support admitted to our hospital from June 2019 to June 2024 were selected as the study subjects and randomly assigned into group A and group B, with 60 cases in each group. Group A underwent subcapsular IOL deep scleral fixation, while group B underwent IOL suture suspension fixation in the ciliary groove. The surgery time, uncorrected visual acuity(UCVA), best corrected visual acuity(BCVA), intraocular pressure(IOP), corneal endothelial cell density(CECD), corneal endothelial cell loss rate, and postoperative complications were compared between the two groups before and at 1, 3, 6 mo after surgery.
RESULTS:The operation time of the group A was lower than that of the group B(24.69±2.69 vs. 32.75±3.75 min, t=11.937, P<0.05). The UCVA and BCVA in both groups were better than those before operation, and the group A was better than the group B(all P<0.05). The loss rates of corneal endothelial cells in the group A were lower than those in the group B at 1, 3 and 6 mo after surgery, the IOP in the group A was lower than that in the group B at 1 mo after surgery, and the CECD in the group A was higher than the group B(all P<0.05). The 3 eyes(5.0%)of the postoperative IOL ectopic in the group A were less than 11 eyes in the group B(18.3%, P=0.023).
CONCLUSION:Subcapsular IOL deep scleral fixation has prominent curative effects on aphakic patients who lack capsule support. It helps improve vision, with less operation time, and fewer postoperative complications.

Abstract:AIM: To investigate the effects of tobramycin dexamethasone eye drops combined with diclofenac sodium eye drops on postoperative inflammatory markers and recurrence in patients with pterygium.
METHODS: A retrospective study was conducted to collect the relevant data of 102 patients(102 eyes)with pterygium who were treated with tobramycin dexamethasone eye drops after pterygium excision and conjunctival transposition of autologous limbal epithelium from May 2021 to May 2024. They were divided into two groups according to different medication, including 51 eyes treated with tobramycin dexamethasone eye drops in control group and 51 eyes treated with tobramycin dexamethasone eye drops and diclofenac sodium eye drops in observation group, and both groups were treated for 4 wk. Followed-up for 6 mo, the levels of inflammatory factors at each time point, ocular symptom scores, lacrimal gland function, adverse reactions during the medication, and disease recurrence were compared.
RESULTS: The time required for conjunctival flap congestion and corneal wound epithelial regression in the observation group was shorter than that in the control group(all P<0.001). At 1, 2, and 4 wk after surgery, the levels of interleukin-6(IL-6), IL-1β, and tumor necrosis factor-α(TNF-α)in both groups decreased compared to the day of surgery, and the levels of each indicator in the observation group were lower than those in the control group at all time points(all P<0.05). At 1 wk after surgery, the time for the first tear film rupture in both groups was shortened compared to before surgery(P<0.05), while there was no statistical significant difference in the Schirmer I test(SⅠt)between the two groups compared to before surgery(P>0.05). At 2 and 4 wk after surgery, the time of first tear film rupture and the SⅠt in both groups increased compared to before and 1 wk after surgery, and the observation group was better than the control group(all P<0.05). There was no significant difference in the incidence of adverse reactions between the two groups of patients during medication(7.8% vs 3.9%, P>0.05). After a 6-month follow-up, the recurrence rate in the observation group was lower than that in the control group, but the difference was not statistically significant(3.9% vs 17.7%, P=0.051).
CONCLUSION: The combination of tobramycin dexamethasone eye drops and diclofenac sodium eye drops for postoperative treatment of pterygium patients is more effective than using tobramycin dexamethasone eye drops alone. It is beneficial for alleviating postoperative inflammatory reactions, improving patient comfort, promoting postoperative lacrimal gland function recovery, reducing recurrence, and has higher safety.

Abstract:AIM:To observe the defocus state and myopia control in myopic children wearing single-vision, defocus, and orthokeratology lenses using multispectral refraction topography(MRT).
METHODS: A total of 279 myopic patients aged 8-14 years old, with a spherical equivalent(SE)from -7.00 to -0.50 D, treated at the Chengdu Aier Eye Hospital from June 2022 to December 2023. Patients who volunteered for the study were assigned to three groups. A total of 94 cases were provided with single-vision spectacle lenses(SVL group), 90 cases received individualized ocular refraction customization(IORC group), and 95 cases received orthokeratology lenses(OK group). Simultaneously, the three groups were further categorized into low(-3.00 to -0.50 D), moderate(-6.00 to -3.25 D), and high myopia(-7.00 to -6.25 D)groups according to different SE. MRT was used to measure and compare the defocus changes of the retina in supperior, inferior, nasal, and temporal quadrants(RDV-S, RDV-I, RDV-N, RDV-T), and three angles of field of view, including 0-15°, 15°-30°, and 30°-45°(RDV-15, RDV-30, RDV-45)in the three groups(the data divide for the connected regions is grouped to the latter group). A one-way analysis of variance was used for intergroup comparisons. Univariate and multivariate linear regression analyses were used to analyze the factors related to changes in the axial length(AL)at 1 a after intervention.
RESULTS:There were significant differences in 1-year SE and AL growth among patients in the SVL, IORC, and OK groups before and after intervention(P<0.001). The 1-year SE and the difference of AL growth in patients with low myopia was significantly different among SVL, IORC, and OK groups(P<0.001); however, there was no significant difference between the IORC and OK groups(P>0.05); there were significant differences in the SE and AL growth changes between the OK group and the IORC and SVL groups in moderate myopia(P<0.001); and there were significant differences between the OK group and the IORC and SVL groups in SE and AL growth of high myopia group after wearing lenses for 1 a(P<0.001), while there were no significant differences between the IORC and SVL groups(P>0.05). In addition, there were significant differences in the relative peripheral refractive errors(RPRE)of 4 quadrants and 3 eccentric regions among the three groups of patients in different degrees of myopia groups(P<0.001). Pair-wise comparison of the growth difference of eccentric D-RDV-15 in low myopia group after wearing lenses for 1 a showed significant differences between the SVL, IORC, and OK groups(P<0.001), but no significant differences between the IORC and OK groups(P>0.05). The angle of field of view D-RDV-30 in moderate myopia subgroups was statistically different between the SVL group and the IORC and OK groups after wearing lenses for 1 a(P<0.001), while the IORC and OK groups showed no significant differences(P>0.05); the angle of field of view D-RDV-15 in high myopia subgroups was statistically different between the OK group and the IORC and SVL groups after wearing lenses for 1 a(P<0.001), but there was no significant difference between the IORC and SVL groups(P>0.05). Univariate and multivariate linear regression model analysis showed that the changes in D-TRVD, D-RDV-45, D-RDV-N, and D-RDV-I correlated with the increase in the difference in 1 a AL.
CONCLUSION: MRT can be used to guide the clinical control of myopia. Myopia development is related to the peripheral retinal defocus state, and the difference of defocus quantity in the inferior nasal side at 30°-45° eccentricity may be a factor regulating the rapid progression of myopia.

Abstract:AIM: To compare the efficacy of simple autologous blood coverage with ILM flap tamponade combined with autologous blood coverage after pars plana vitrectomy(PPV)with internal limiting membrane(ILM)peeling in treating macular hole retinal detachment(MHRD)of high myopia patients with moderately long axial length.
METHODS: This retrospective study enrolled 45 patients(45 eyes)with high myopia and MHRD, and axial lengths of 26-29 mm treated at our institution between January 1, 2020 and January 1, 2024. Patients were divided into two groups based on surgical technique: group A(24 eyes)underwent PPV with ILM peeling, ILM flap tamponade, autologous blood coverage, and silicone oil injection; group B(21 eyes)received PPV with ILM peeling followed by autologous blood coverage and silicone oil injection. Intraocular pressure, best-corrected visual acuity(BCVA), retinal reattachment and macular hole closure status were compared at 1 wk, 1, 3, and 6 mo postoperatively. Silicone oil removal was performed at 6 mo postoperatively, with additional 2-month follow-up.
RESULTS:At 8-month postoperative follow-up, both groups achieved complete retinal reattachment. Macular hole closure rates showed no significant intergroup difference(88% vs 86%, P=0.860). Significant improvement in BCVA was observed in both groups compared to preoperative values, with the group B demonstrating better visual outcomes than the group A(P<0.001). Transient parafoveal subretinal fluid persistence was noted in 2 eyes of the group A(resolved spontaneously at 5 and 8 mo post-PPV, respectively), and 1 eye of the group B(resolved by 4 mo post-PPV). Serial optical coherence tomography(OCT)monitoring revealed no macular hole reopening, with complete subretinal fluid resolution confirmed in all cases at final follow-up.
CONCLUSION:For high myopic MHRD patients with moderately long axial length, both surgical approaches effectively promote macular hole closure and retinal reattachment. However, the autologous blood coverage technique demonstrates better BCVA than the combined ILM tamponade.

Abstract:AIM: To investigate the effect of the relationship between scotopic pupil and optical zone diameters on short-term subjective and objective visual quality after small incision lenticule extraction(SMILE).
METHODS:In this prospective cohort study, 98 patients(196 eyes)who underwent SMILE from September 2021 to June 2023 were included. Participants were divided into two groups based on the ratio of scotopic pupil diameter to optical zone diameter: group A(ratio >1, 70 eyes)and group B(ratio ≤1, 126 eyes). The preoperative and postoperative uncorrected visual acuity(UCVA), spherical equivalent(SE), total corneal high-order aberrations at 4, 6, and 8 mm of pupil diameters, objective scatter index(OSI), pre- and post-operative QoV subjective visual quality questionnaire were observed and recorded. The refractive status of different groups of patients at different time points before and after surgery, and differences in subjective and objective visual quality indices were analyzed. Furthermore, the changes in subjective and objective visual quality(postoperative-preoperative)at different postoperative time points were analyzed between the two groups.
RESULTS:No significant differences in visual acuity or refractive state were observed between the two groups at 3 mo postoperatively. In both the group A and the group B, there was a difference in the changes of corneal total higher-order aberration centered on 8 mm cornea at 1 mo postoperatively(P<0.05), and there was a difference in the changes of total higher-order aberration and corneal spherical aberration centered on 8 mm cornea at 3 mo postoperatively(all P<0.05). At 3 mo after surgery, the most commonly reported symptoms in the group A were glare, starburst, hazy vision, and halo. In the group B, the most common symptoms were hazy vision, halo, starburst, and glare. Statistically significant differences were observed in the severity of glare and visual fluctuation between groups before surgery and at 3 mo postoperatively(all P<0.05). However, no significant differences were found in the severity of halo, starburst, blurred vision, double vision, or focusing difficulty at 3 mo postoperatively(all P>0.05).
CONCLUSION:When the scotopic pupil diameter exceeds the optical zone, SMILE may increase postoperative corneal aberrations, as evidenced by an increase in high-order corneal aberrations within an 8-mm central corneal range, a higher incidence of postoperative glare, and more severe glare and visual fluctuation symptoms. Nevertheless, these symptoms are mild and remain within a safe range.

Abstract:AIM: To assess the effectiveness of selective laser trabeculoplasty(SLT)in the treatment of primary open angle glaucoma(POAG).
METHODS:Totally 24 patients(33 eyes)with POAG who had poor control of intraocular pressure(IOP)from June 2022 to December 2023 were included in this retrospective study, and all of them were treated with SLT. Furthermore, the IOP, mean deviation(MD), mean sensitivity(MS), and the thickness and vascular density(VD)of optic disc nerve layer were compared after treatment.
RESULTS: There were significant reducion in IOP at 1, 3, 6, 12 mo after treatment(all P<0.001). There were no statistically significant differences in MD and MS before treatment and at 6 and 12 mo after treatment(all P>0.05), and there were no statistical significant differences in the thickness and VD of optic disc(all P>0.05).
CONCLUSION: SLT can significantly reduce IOP in POAG patients in the short term, without damaging the visual field and optic disc nerves, while the effectiveness of SLT decreases over time.

Abstract:AIM: To discuss the expression and diagnostic value of serum trefoil factor 1(TFF1)and annexin A2(ANXA2)in diabetic retinopathy(DR).
METHODS:A total of 50 DR patients(100 eyes; research group)and 50 type 2 diabetes mellitus(T2DM)patients(100 eyes; control group)without retinopathy who visited our hospital between January 2022 and December 2024 were recruited. Enzyme linked immunosorbent assay was performed to test serum TFF1 and ANXA2. Pearson method was performed to analyze the correlation between serum TFF1 and ANXA2 in DR. Logistics method was used to discuss the influencing factors of retinopathy in T2DM patients. ROC was performed to analyze the diagnostic value of serum TFF1 and ANXA2 in DR.
RESULTS: The research group had clearly longer T2DM course, higher glycated hemoglobin, total cholesterol(TC), and serum ANXA2 than the control group(all P<0.001); the research group had significantly lower serum TFF1 than the control group(P<0.001); Serum TFF1 and ANXA2 in DR patients were negatively correlated(r=-0.345, P=0.014). The expression of serum TFF1 and ANXA2 was influencing factor for the occurrence of retinopathy in T2DM patients(all P<0.05). The AUC of serum TFF1 in diagnosing DR was 0.831(95%CI=0.746-0.916), and the AUC of serum ANXA2 in diagnosing DR was 0.832(95%CI=0.755-0.908). The combination of the two had an AUC of 0.932(95%CI=0.884-0.980)for diagnosing DR. The combination of serum TFF1 and ANXA2 had a higher diagnostic value for DR than individual diagnosis(Zcombination-TFF1=2.883, P=0.004; Zcombination-ANXA2=2.947, P=0.003).
CONCLUSION: Serum TFF1 is reduced and serum ANXA2 is increased in DR patients. These changes are associated with the increase in risk of retinopathy in T2DM. The combination of serum TFF1 and ANXA2 has certain diagnostic value for DR.

Abstract:AIM: To investigate the risk factors for dry eye syndrome secondary to type 2 diabetes mellitus(T2DM), and to develop a nomogram model for early risk prediction.
METHODS: A total of 347 T2DM patients treated in our hospital between March 2020 and April 2024 were enrolled and randomly divided into training(242 cases)and validation(105 cases)sets at a 7:3 ratio. Demographic data, glycemic parameters, and clinical treatments were compared between non-dry eye syndrome(control group, 86 cases)and dry eye syndrome to type 2 diabetes mellitus(dry eye group, 156 cases)in the training set. Statistically significant indicators were incorporated into multivariate Logistic regression to identify risk factors for secondary dry eye. These factors were then used to construct a nomogram model using R software, which was subsequently validated using the validation set.
RESULTS:The percentage of patients with secondary dry eye syndrome in 242 cases of T2DM was 64.5%(156/242). Multifactorial Logistic regression revealed that blood glucose variability, glycosylated serum protein, retinopathy, meibomian gland functional status, duration of T2DM, and meibomian gland opening blockage were the risk factors for secondary dry eye(all OR>1, P<0.05). A nomogram prediction model was constructed based on the 6 indicators above, and the area under the receiver operating characteristics(ROC)curve of the training and validation sets was verified to be 0.994(95%CI: 0.989-0.999)and 0.990(95%CI: 0.977-0.999), respectively. The slopes of the calibration curves were similar, as tested by the Hosmer-Lemeshow test χ2=1.461, 1.566, P=0.993, 0.992. The nomogram model could provide good utility for clinical decision-making.
CONCLUSION:Glycemic variability, glycated serum protein, retinopathy, meibomian gland dysfunction, T2DM duration, and meibomian gland orifice obstruction significantly increase the risk of dry eye secondary to T2DM. The constructed nomogram model serves as a valuable tool for early risk assessment and intervention, and it is helpful for early diagnosis and intervention, thus improving patients' quality of life.

Abstract:AIM: To investigate the clinical efficacy of pars plana vitrectomy(PPV)combined with fovea-sparing internal limiting membrane(ILM)peeling and silicone oil(SO)tamponade for treating pathological myopic foveoschisis(PMF).
METHODS:This study is a retrospective observational analysis of 10 cases(10 eyes)diagnosed with PMF that underwent PPV with fovea-sparing ILM peeling and SO tamponade between January 2023 and November 2024. The best-corrected visual acuity(BCVA), central foveal thickness(CFT), foveoschisis(FS), and the detachment and reattachment of FS and macular fovea were assessed preoperatively and at 1 wk, 1 and 3 mo postoperatively.
RESULTS:Among the 10 cases of PMF patients(10 eyes), the complete reattachment rate was 30%(3 eyes), while partial reattachment was observed in 70%(7 eyes). At 3 mo postoperatively, BCVA(LogMAR)was significantly improved to 0.957±0.393 compared with 1.432±0.509 before surgery(P<0.05), and both CFT(437.9±180.4 vs. 207.5±76.1 μm)and FS(686.5±172.2 vs. 290.7±86.6 μm)showed significant decreases(P<0.05). No complications such as macular hole, retinal detachment, silicone oil emulsification, or endophthalmitis were observed during the surgery or throughout the follow-up period.
CONCLUSION:PPV with SO tamponade and fovea-sparing ILM peeling has been demonstrated to facilitate both visual acuity improvement and anatomical reattachment in cases of PMF.

Abstract:AIM: To investigate the risk factors of myopia, and provide evidence for the prevention and treatment of myopia in preschool children.
METHODS:This is a retrospective case-control study. A total of 168 preschool-aged children(168 eyes)were enrolled from the ophthalmology department at Affiliated Hospital of Qingdao University between September 2021 and September 2023, selecting the eye with poorer vision for inclusion; the right eye was selected when both eyes had equal vision. Grouping criteria: Children with spherical equivalent(SE)>0 D and age-appropriate normal visual acuity(≥0.5 for ages 3-5; ≥0.7 for ages 6-7)were directly assigned to the non-myopia group. Children with SE <0 D underwent cycloplegic refraction after 3 days of 1% atropine sulfate gel application(three times daily). Those with SE ≤-0.50 D and failure to achieve age-normal visual acuity(<0.5 for ages 3-5; <0.7 for ages 6-7)were assigned to the myopia group, with 84 children(84 eyes)in each group. All participants underwent measurements of visual acuity, intraocular pressure, refraction, and ocular biometric parameters. Parents or guardians completed risk factor questionnaires during the visit, covering daily outdoor activity duration, daily electronic device usage/reading time, parental myopia status, maternal delivery mode, and full-term birth status.
RESULTS:Significant differences were observed in uncorrected visual acuity, spherical power, SE, and axial length between the two groups of children(all P<0.05). No statistically significant differences were found in daily sleep duration, daily reading/homework time, maternal myopia status, mode of delivery, or full-term delivery status(all P>0.05). However, significant differences existed in daily outdoor activity time, daily electronic device usage duration, maternal myopia degree, paternal myopia degree, maternal age of myopia onset, and paternal age of myopia onset(all P<0.05). Logistic regression analysis identified the following risk factors for myopia in preschool children: daily outdoor activity under 2 h, daily electronic device use exceeding 60 min, maternal myopia degree, maternal early-onset myopia, paternal high myopia, and paternal early-onset myopia. Analysis of interaction effects children gender and myopia-related factors on SE revealed significant interactions of children gender with paternal myopia degree, maternal and paternal age of myopia onset(P<0.05). Simple effects analysis further demonstrated that girls exhibited higher degree of myopia than boys in families with paternal early-onset myopia, maternal early-onset myopia, or paternal high myopia(P<0.05).
CONCLUSION:Insufficient daily outdoor activity time(under 1 h), daily electronic device use exceeding 60 min, and parental myopia are significant risk factors for myopia development in preschool children. Among these children, girls' refractive status was particularly associated with parental refractive status.

Abstract:AIM: To explore the relationship between hyperopia reserve and ocular biometric parameters in 4-14 year-old Uyghur students from Hotan County, Xinjiang, and to provide scientific evidence for myopia prevention.
METHODS: From September 1 to October 31, 2023, a stratified random cluster sampling method was used to select 3 264 students(3 264 eyes)from 6 schools in Hotan County. Participants underwent uncorrected distance visual acuity testing, cycloplegic refraction, and ocular biometric measurements. The correlation between spherical equivalent(SE)and ocular biometric parameters was analyzed by multiple linear regression.
RESULTS: A total of 1 998 non-myopic students(1 998 eyes)were included in the study, with 1 354 students(67.77%)showing insufficient hyperopia reserve. The detection rate of insufficient hyperopia reserve decreased with age, from 94.12% at age 4 to 18.13% at age 14(P<0.001). Multiple linear regression analysis showed that in the group with sufficient hyperopia reserve, age, gender, uncorrected distance visual acuity, axial length(AL), and keratometry(K)explained 66.5% of the variance in SE; while in the group with insufficient hyperopia reserve, these factors explained only 28.0% of the SE variance.
CONCLUSION: In non-myopic Uyghur students aged 4-14 in Hotan County, Xinjiang, the detection rate of insufficient hyperopia reserve was 67.77%. In the group with insufficient hyperopia reserve, age, gender, AL, and K explained only a small portion of the SE variance, suggesting that the refractive status of this population may be influenced by more complex factors.

Abstract:AIM: To analyze the recovery of stereopsis and its influencing factors in children with intermittent exotropia(IXT)after binocular vision training.
METHODS: A total of 166 cases of IXT children who were treated in our hospital from October 2021 to October 2023(2 cases lost their follow-up, and 164 cases were finally included)were included as the research object, taking 36 cases with no stereopsis after binocular vision training in eye position correction surgery as no stereopsis group, and other 128 cases as stereopsis group. All the children underwent eye position correction surgery under general anesthesia, and all received binocular vision training for 6 mo after surgery. The recovery of stereoscopic vision of IXT children after binocular vision training was counted, and the influencing factors of stereoscopic vision recovery of IXT children after binocular vision training were analyzed by single factor and multi-factor Logistic regression analysis.
RESULTS: The incidence of postoperative no stereopsis was 22.0%. The proportion of children with an age ≥9 years old, course of disease ≥1 a and anisometropia in the group without stereoscopic vision after operation was larger than the group with stereoscopic vision(all P<0.05). Multivariate Logistic regression analysis showed that the course of disease ≥1 a, age ≥9 years old and anisometropia were independent influencing factors for the recovery of stereoscopic vision in IXT children after binocular vision training(OR=1.470, 1.626, 1.539, all P<0.05).
CONCLUSION: Age ≥9 years old, course of disease ≥1 a, and anisometropia are the independent influencing factors of stereopsis recovery of IXT children after binocular vision training. Therefore, targeted intervention measures can be given to high-risk children to improve the stereopsis recovery of IXT children after binocular vision training.
