探究血府逐瘀丸联合阿柏西普治疗视网膜静脉阻塞(retinal vein occlusion, RVO)的疗效以及对视网膜微循环、黄斑水肿(macular edema,ME)的影响。方法:选取2021年1月至2024年3月期间我院收治的80例RVO患者作为受试者,按照治疗方法将其分为对照组40例(阿柏西普治疗)和观察组40例(血府逐瘀丸联合阿柏西普治疗),比较两组患者治疗RVO的疗效、中医证候积分、视网膜微循环参数、ME以及不良反应。结果:观察组治疗后的临床有效率为95.00%,明显高于对照组(80.00%)(P<0.05);治疗后,两组患者中医证候积分、黄斑中心凹无血管区(foveal avascular area, FAZ)面积、FAZ周长、黄斑中心视网膜厚度以及新生血管渗漏面积均有降低,整体视网膜浅层毛细血管丛(superficial capillary plexuses, SCP)血流密度以及视网膜深层毛细血管丛(deep capillary plexuses, DCP)血流密度均有上升,观察组结果优于对照组(P<0.05);观察组药物不良反应总发生率(20.00%)明显低于对照组不良反应总发生率(42.50%)(P<0.05)。结论:血府逐瘀丸联合阿柏西普治疗RVO可改善视网膜微循环,提高视力,恢复眼部血液循环,改善出血情况,促进ME吸收,提高临床疗效。

Objective: To investigate the efficacy of Xuefu Zhuyu pill combined with aflibecept in treating retinal vein occlusion (RVO) and its impacts on retinal microcirculation and macular edema (ME). Methods: Select 80 RVO patients admitted to our hospital from January 2021 to March 2024 as subjects. According to the treatment method, they were assigned into a control group of 40 cases (treated with aflibercept) and an observation group of 40 cases (treated with Xuefu Zhuyu pill combined with aflibercept). The efficacy, TCM syndrome score, retinal microcirculation parameters, ME, and adverse reactions of the two groups were compared. Results: The clinical effective rate of the observation group after treatment was 95.00%, obviously higher than that of the control group (80.00%) (P<0.05). After treatment, the traditional Chinese medicine syndrome scores, foveal avascular area (FAZ) area, FAZ circumference, macular central retinal thickness, and neovascularization leakage area of both groups decreased, the overall blood flow density of the SCP and deep capillary plexuses (DCP) increased, and the observation group showed better results than the control group (P<0.05). The total incidence of adverse reactions in the observation group (20.00%) was obviously lower than that in the control group (42.50%) (P<0.05). Conclusion: The combination of Xuefu Zhuyu pill and aflibecept can improve retinal microcirculation, enhance vision, restore ocular blood circulation, improve bleeding, promote ME absorption, and improve efficacy in the treatment of RVO.
