目的:探究有晶体状眼后房型人工晶体ICL植入术后拱高与高阶像差HOA,包括全眼、眼内及角膜高阶像差之间的相关性。 方法:回顾性研究。收集自2020年1月至2024年12月于苏州大学附属第三医院眼科顺利行ICL手术的患者46人共91只眼,收集术后超过3个月的视力VA,眼压IOP,等效球镜度数SE,拱高,前房深度(ACD),角膜内皮细胞计数及眼部像差。采用Spearman相关性检验对拱高与各高阶像差进行相关性分析,利用配对样本t检验比较术前与术后IOP、ACD和角膜内皮细胞计数之间的差异。 结果:拱高与眼部部分高阶像差存在显著相关关系。角膜总HOA,角膜球差,角膜彗差及角膜三叶草差均与拱高具有显著负相关关系(p<0.05),眼内球差与拱高具有显著正相关关系(p=0.016)。与术前相比,术后IOP,ACD及角膜内皮细胞计数均呈下降趋势,但差异无统计学意义。 结论:拱高通过影响角膜各高阶像差从而进一步影响视觉质量;ICL植入术总体是安全有效的。

[Abstract]
Purpose: To investigate the correlation between vault and higher-order aberrations after EVO implantable collamer lens (ICL) implantation. Methods: A retrospective study. A total of 91 eyes of 46 patients who underwent ICL surgery in the First People’s Hospital of Changzhou from January 2020 to December 2024 were collected. VA, IOP, SE, vault, ACD, the number of corneal endothelial cells and ocular aberrations were collected more than 3 months after surgery. Correlation analysis between vault and high-order aberrations using Spearman correlation test.The differences of IOP, ACD and the number of corneal endothelial cells before and after surgery were compared using paired sample t-test. Results: Significant correlations existed between the vault and higher-order aberrations in the ocular system. All of corneal higher-order aberration components (total HOA, spherical aberration, coma aberration, and clover aberration) exhibited statistically significant inverse correlations with ICL vault values(p<0.05). Intraocular spherical aberration was positively correlated with vault(p=0.016). Compared with those pre-operation, IOP, ACD and the number of corneal endothelial cells showed a decreasing trend after operation, but the difference was not significant (p>0.05). Conclusions:Vault further affects visual quality by affecting corneal higher-order aberrations. ICL implantation is generally safe and effective.
