圆锥角膜是一种以角膜曲率增大、进行性变薄和前突为特征的角膜扩张性疾病。患者在疾病早期可出现进行性近视和不规则散光,随着病情进展,屈光不正加重,导致严重视力障碍。角膜交联术(CXL)是目前唯一有效控制圆锥角膜进展的手段。角膜上皮重塑是指角膜基质曲率变化时,角膜上皮细胞通过再生长和重新分布来维持角膜表面的光滑性和对称性。角膜上皮重塑对角膜屈光术后视力恢复和角膜形态维持均具有重要作用,但对角膜交联手术效应的作用尚不完全清楚。本文综述了角膜交联术后上皮重塑的特点及其临床意义,不同交联方案对角膜上皮厚度的影响,以及上皮重塑在术后疗效评估和手术方案改良中的潜在价值。

As a corneal ectatic disease, keratoconus is characterized by increased corneal curvature, progressive thinning, and anterior protrusion of the cornea. Initially, patients often experience progressive myopia and irregular astigmatism. Progression leads to worsening refractive errors and significant visual impairment. Corneal collagen cross-linking (CXL) is currently the only effective treatment to halt keratoconus progression. Corneal epithelial remodeling refers to the regeneration and redistribution of epithelial cells to maintain corneal surface smoothness and symmetry following changes in stromal curvature. While this remodeling process significantly contributes to visual recovery and corneal shape maintenance after refractive surgeries, its role in mediating the effects of CXL treatment remains incompletely understood. This review comprehensively summarizes the characteristics and clinical significance of corneal epithelial remodeling following CXL, the impact of different CXL protocols on epithelial thickness, and the potential value of epithelial remodeling in assessing postoperative efficacy and optimizing surgical strategies.
